Affinity

The helpfile suggests this spell is permanent unless you die while not carrying the item you've placed Affinity on. However, this is not the case. Anytime you die, Affinity wears off. It still functions and returns your item to you, but you must re-apply affinity after each death for it to work again.

Another minor issue is there is no way to know what item you've placed affinity on if you somehow forget. It would be nice if the item showed up with (Spiritbound) or something in your inventory.
